New Owner Paperwork

Congratulations on becoming a new Access owner! We appreciate your trust in us and will work hard to keep your home safe and you happy. Below are a few forms that need to be filled out to get started. In addition to these forms, you will be emailed the W-9 and ACH payment paperwork through Adobe sign. You can rest easy knowing that each document is encrypted and can be validated through a trusted third-party certificate authority. Once each form is completed by all parties, a copy will be emailed to you for your records.

Management Forms

This includes our management agreement, bookkeeping form, and the fee schedule. The fee schedule goes over the current pricing structure for Access’ services. Please note: the specific fees that apply to your property are detailed on the bookkeeping form.

The bookkeeping form goes over basic information and specific pricing for your account.

Fill out the ACH and W-9 so we can distribute your owner funds.

The management form packet includes the termination of agreement addendum, rent reduction addendum, insurance form, lead based paint disclosure, and the utility service authorization form.

The property information questionnaire will help provide all the details we need to know to market and manage your property. Expect it to take 15-30 minutes to complete.


Utility Authorization Forms

Some cities or utility companies require different forms to authorize service in-between tenants. Please fill out all applicable authorization forms for your rental property. If you have a question about which forms to use for your property, please contact barbara.edie@mainlander.com.

This form is applicable for properties within Hillsboro city limits.

This form is applicable if your property uses PGE.

This form is applicable if your property uses NW Natural.

This form is applicable if your property uses Pacific Power.
